Q: Can I make an appointment or walk in for shots?

A: The Immunization Program does not give shots. Contact your medical provider to seek immunization services.  If you do not have a medical provider, you can also contact the Northern Community Health Center (671) 635-4410/7400 or Southern Community Health Center (671) 828-7604/05.

 

 

Q: What are the required documents to request for a copy of my child’s shot record?

A:
  • Child’s Birth Certificate
  • Parent/Guardian Photo ID
  • Letter of Authorization or Legal Guardianship document

 

 

Q: What are the required documents to request for an official immunization record for social security?

A:
  • Child’s Birth Certificate
  • Parent/Guardian Photo ID
  • Letter of Authorization or Legal Guardianship document
  • Child’s Shot Record

 

 

Q: What immunizations are required for school enrollment?

A:
  • All students must be up to date with all required immunizations prior to being enrolled in any school within Guam. Minimum requirements include: DTaP, IPV, MMR, Hep B, Hib* (*for Headstart students only). Students must also receive all follow-up immunizations needed to complete the series of each vaccine.
  • However, we highly encourage that children receive all recommended vaccines in accordance with current the ACIP Schedule.

 

 

Q: How much does a copy of the shot record cost?

A: As for now, the Immunization Program does not charge for the copy of the shot record

 

 

Q: When is the next immunization outreach?

A: Outreaches will be announced on the media, on our website and our programs Facebook.

 

 

Q: Without medical insurance how can I get my children immunized?

A:
  • The Vaccines for Children (VFC) Program is a federal program that provides eligible children with all recommended vaccines at no cost. There is no charge for any vaccines given by a VFC provider to eligible children. But there can be some other costs with immunization:
    • Doctors can charge a set (or standard) fee to administer each shot.
    • There can be a fee for the doctor’s/office visit.
  • Children from birth through 18 years of age can receive VFC vaccine if they are:
    • enrolled in Medicaid
    • uninsured
    • underinsured
    • American Indian or Alaskan Native as defined by the Indian Health Services Act (25 U.S.C. 1603)

  • Information for travelers can be found on the Centers for Disease Control and Prevention’s (CDC) travel website. These pages have updated information regarding travel notices and immunization recommendations.
