For JFK students & parents/guardians

 

Participants must submit a consent form prior to receiving the vaccine. Printed copies are available at JFK. An online PDF form is available at https://tinyurl.com/3xvaw4b3. 

Participants may schedule online appointments https://tinyurl.com/2bjt2dwc. Participants will be instructed to enter through the A-wing and directed to exit through the gymnasium after they complete their observation period. To preserve the gymnasium court, participants are asked to refrain from wearing stilettos or high heels. For information, email matfranquez@gdoe.net.
